1 Completion the HealthCard application form

• Complete the HealthCard application form in black ink.
• Attached the ID copies for both principle and secondary HealthCard applicants.

2 Submission of completed HealthCard application form

• Submit the originally completed application form accompanied by the relevant ID copies.
• Originally completed application forms can be submitted by:
  • Post
  • Delivery to any of the Resolution branches
• Click on the following link to obtain submission details

3 Distribution of HealthCards and PIN numbers

• HealthCards and PIN numbers are distributed within 5 to 7 days from receipt of the originally completed application form.
• HealthCards are posted via registered post and available for collection at the members’ nearest post office, while PIN numbers are posted via normal post.
