| Dear HealthCARD holder, As the year is drawing to a close, we are pleased to inform you that the credit balance available on your HealthCard can be transferred to your personal bank account by 12 December 2008. There will, however, be a minimum balance of R 200 retained to continue the account. Should you wish to make use of this offer, please download the withdrawal form here. Completed forms must be submitted by latest Friday 14 November 2008. Forms can be sent via email or fax:
Group members whose HealthCards are funded by employers are not eligible for this withdrawal, unless it is approved by the employer. Please note that the current available balance will automatically roll over to the next year should you not make use of this offer.
